For Support Coordinators
We work with support coordinators to get participants in for therapy and assessments, with plan-ready reports.
What we offer participants
- Speech pathology and occupational therapy for all ages, in clinic, at home, at school or via telehealth
- Functional capacity assessments (FCA), SDA / SIL / ILO assessments, home modification and assistive technology assessments
- Reports written to the NDIS templates, formatted for planning meetings and plan reviews
- Support alongside an autism or ADHD assessment pathway, and post-stroke and adult rehabilitation
How to refer
Email admin@spacetogrow.au or use the referral form with the participant’s details, plan management type, NDIS number and goals. We will confirm current capacity and likely wait, book an intake, and send a service agreement. For plan- and agency-managed participants we invoice the plan manager or the NDIA directly.
What you can expect from us
- A named therapist and a clear point of contact
- An initial assessment summary and periodic progress reports (with consent)
- Reports delivered to agreed timeframes, with enough notice if something will slip
- A call-back when you need to discuss a shared participant
