Information we collect
We collect the information needed to provide safe, effective allied health care — for example name and contact details, date of birth, funding details (such as NDIS or Medicare numbers), referral information, and clinical notes, assessments and reports created during your care.
How we use it
- To provide and coordinate your speech pathology or occupational therapy
- To communicate with you and, with your consent, with referrers, funders and other treating professionals
- To bill for services and meet our legal and professional obligations
Storage and security
Records are held in secure electronic systems with access limited to staff involved in your care. We retain records for the period required by law and dispose of them securely afterwards.
Access, correction and complaints
You can ask to see or correct the information we hold about you by contacting the practice. If you have a privacy concern, contact us first; if it is not resolved you may contact the Office of the Australian Information Commissioner.
