What the assessment looks at
- Everyday tasks — showering, dressing, cooking, getting around the home
- Falls risk and what is contributing to it
- Equipment that would make daily tasks safer or easier
- Home set-up — access, bathroom, lighting, trip hazards
- Support to keep doing valued activities, not just the essentials
How it works with your package
Your Home Care Package provider can bring us in to complete the assessment. We provide a clear report with prioritised recommendations your provider can action — equipment to order, minor modifications, referrals, and strategies for you and any support workers.
After the assessment
Where a recommendation needs more work — a complex home modification, a piece of prescribed equipment — we can carry out that follow-up assessment too, so you are not passed between providers.

I do not have a package yet — can you still help?
Yes. We see older adults privately and can complete an assessment while a package is being arranged. Ask about fees when you call.
Can family be there?
Absolutely — it often helps to have someone who knows your routines in the room.
